GDPR Statement

As the patient of New-Smile Dent Ltd. (8800 Nagykanizsa, Ady Endre street 12) (hereinafter referred to as Data Controller), voluntarily, following appropriate information, I make the following data management consent statement.

On the basis of my present statement, I consent until withdrawal to the fact that the Data Controller, on the basis of the conditions set out in its Privacy Policy, the Data Controllers contained therein may manage my personal data (identification and contact details: name, telephone number, e-mail address, other identifiers) and my health data directly necessary for my care to the extent necessary for the performance of their tasks relating to the services provided by the Data Controller. I understand that the processing of data on the basis of my consent contained in this Statement is subject to the Policy and the applicable data protection laws, in particular the EU 2016/679 General Data Protection Regulation (GDPR), Info Law on Informational Self-Determination and Freedom of Information, AAct CLIV of 1997 on Health and the provisions of Act XLVII of 1997 on the Processing and Protection of Medical and Other Related Personal Data.
